Abstract:
The opioid system of the brain is the major target for opiate drugs such as morphine and heroin, and has been implicated in processes such as pain, stress and reward. Many of these effects take place at the mu opioid receptor (mOR), which is distributed throughout the brain. Significantly, genetic polymorphisms at the mOR may confer a greater dopamine response to the reinforcing effects of alcohol, and it has been suggested that addiction per se may be associated with alterations to the opioid system. There is evidence for the potential efficacy of mOR antagonists (e.g. naltrexone) in reducing

Abstract:
Given the complex and chronic medical problems that are seen in the palliative care setting, there is an ever increasing need for awareness of prescription drug abuse. Providers must balance the potential for abuse of prescribed opioids with the need to provide appropriate analgesia for patients in the palliative care setting. In addition, the presence of aberrant drug use amongst patients with advanced illness represents a major impediment to appropriate care. In order to maximize patient outcomes and to prescribe needed medication both safely and fairly, the clinician should work to develop

Abstract:
This chapter describes the dilemma of today’s chronic pain patient in the face of well-intentioned regulatory efforts to reduce opioid-related mortality. From the beginning of recorded history, there has been interest in substances derived from opium poppy. As modern governments evolved, efforts were made to ensure the availability of opiates for medicinal use while restricting their nonmedical use. This chapter discusses US efforts to control opiates and the severe problem of opiate abuse in the United States that gave rise to these efforts. The United States was the first nation to establish

Abstract:
Prescription Drug Diversion and Pain provides an interdisciplinary overview of medications used to treat chronic pain, and the benefits and risks that are posed by long-term use of opioids. Use of these life-saving medications must be carefully managed to prevent serious side effects, which may include physical dependence, addiction, and even death. In recent years, the risk of these side effects has led to increased attention on the development of alternative treatments for chronic pain. This book not only offers a single, comprehensive source for understanding the specialized nature of the o
