This case explores how bone health can be affected by antiepileptic drugs (AEDs), and why this is an important issue in women with epilepsy (WWE). AEDs affect bone health through activation of the cytochrome P450 system in the liver, leading to increased metabolism of vitamin D. The enzyme-inducing AEDs include phenytoin, carbamazepine, phenobarbital, and primidone. Risk factors for osteoporosis include female gender, low body mass, inadequate vitamin D intake, and smoking. Back pain is prevalent in adults, and most often its cause is nonspecific and benign. Imaging and interventions are not always helpful and they are generally expensive and low yield. When Harold Schuknecht arrived at Harvard in 1961, he immediately set up a temporal bone laboratory and began collecting specimens. Schuknecht obtained two specimens from patients with a typical clinical picture of benign paroxysmal positional vertigo (BPPV). In these specimens, Schuknecht identified a prominent granular basophilic staining mass attached to the cupula of the left posterior semicircular canal.
